The issue reproduces for me on Windows version v1.16.201 if connected to Realms or to a LAN server.
Steps:
run the Windows version from Microsoft Store
connect to Realms or LAN server
minimize the game window
unminimize the game window (don't even need to wait before that)
Observed result: you get disconnected from the server
